Join us to learn about the Unite Washington network and how we're connecting our community to resources! This community strategy session will share information about Unite Us, our work in Washington, a software demo, and how to participate in the network. You should register for this session if you are interested in learning about the network and your organization has not yet joined. Unite Washington is a coordinated care network of health and social care providers. Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Washington is sponsored by a variety of funding partners currently led by the Community Health Plan of Washington, Kaiser Permanente, OptumCare, and MultiCare.
